Trinity Assistance's Medicaid Service Coordination Program helps people with developmental disabilities gain access to many services that will address their specific needs and personal goals. The Medicaid Service Coordinator (MSC) provides case management services to assist individuals with accessing a variety of services including: guardianship, respite, recreation, family reimbursement, and securing safe and affordable housing. Once an Individualized Service Plan (ISP) is developed by the MSC with the individual, it is then monitored, and evaluated to support individuals as they move towards accomplishing their goals and achieving a greater level of independence. Access to the Medicaid Service Coordination programs begins with the intake process. In order to apply for Medicaid Service Coordination the following is needed:
- Active Medicaid
- Approved eligibility to receive services through Office of People with Developmental Disabilities (OPWDD) http://www.opwdd.ny.gov/
- Ongoing Comprehensive Need for services
- Verification of engaging in the OPWDD Front Door Individual and Community Support (ICS) Information Session
