Job description: Management of Reportable Incidents and Allegations of “Abuse,” and all unusual Events; including typing of reports and maintaining secured storage of documents, Coordinator of Incident Review Committee (IRC) meetings, Complete investigations per policy, updates all P&P pertaining to QA; Ensures all P&P are in accordance with State Regulations. Responsible for accurate completion of internal audit functions and Policy and Procedures updates/revisions. Acts as a resource person within the agency for incident management and as a liaison with the OPWDD for incident management, attend any OPWDD trainings which may come up and come back and report any changes to the CEO. Responsible for accurate completion of internal audit functions and Policy and Procedures updates/revisions for all Trinity Programs. Develops and maintains the record retention system, coordinates the provision of records in response to outside audits by oversight agencies.
Requirements: Bachelor’s Degree in Human Services / Business Administration, Psychology or Accounting. Minimum 3 years experience working with people with Developmental Disabilities & Procedures. Knowledge of Mental Hygiene Laws & NYS Regulations pertaining to Department of Health; 633, 624 & 625 Regulations for incident reporting to DDRO and the monitoring of IRMA incident closures, Experience in Investigative Reporting; class requirements include OPWDD Incident Management Training Certificate, How to complete a thorough investigation and conclusion. Knowledge of Microsoft Office and navigate the internet independently; ability to learn new programs through video conferences for applications such as IRMA and Office 365/Share Point/ADP, CHOICES

Computer competency required. Services will be provided in the family home and in the community. Looking for daytime and evening hours – can be full or part-time based on staff’s and individual’s needs. Bachelors/Associates or prior experience preferred. Rate to start is $13/hour with the possibility of a raise after a trial period.

An ability to build and maintain positive relationships with individuals receiving waiver services, their families/advocates, staff, and community. Must be highly organized, Skilled in using computer for email communications. Microsoft Office (Word, Excel), quick learner with new computer program software that is required for day to day program operations.
