Individualized Support Services (ISS) are available to assist adults with establishing their own household or independent living setting by renting or owning their own home. ISS applies to individuals that want to leave a Certified or Institutional Setting, those who are ready to move from a family home, or are unsuccessfully living on their own and have significant health and safety issues identified, face eviction, loss of housing or who are homeless. ISS is not able to assist individuals who live at home with biological, adoptive or surrogate family members.

Per OPWDD, all new ISS requests will be authorized against the following Health and Safety Criteria:

* Living situation presents as abusive or neglectful constituting imminent risk of harm
        - Domestic Abuse
        - APS/CPS/Court involvement
* Homeless or in imminent danger of becoming homeless
        - Living in a shelter
        - Living on the streets
        - Facing eviction due to circumstances not caused by the tenant
*Aging or failing health of caregiver/no alternate caregiver available
*Medical/Physical condition requires alternate living arrangements
*Psychiatric Admissions/discharges in the community
MSCs will work with the individual, ISS Liaison and DDRO to seek funding approval and complete/obtain all supporting documentation applicable.
